To the TLBCers out there:

I was suppose to get a medical info paperwork package by courier today.
It didn't arrive. Now I have to wait until Monday or later. (I had hoped to do the lab work tomorrow.)
What is in it??

Apr. 09/08 - Phone Consult, Apr. 14/08 - Labwork & ECG
May 22/08 - Banded (self-pay) by Dr. Yau @ TLBC
Aug. 11/08 - 1st Fill @ 3cc

I am not sure, but I beleve the just do Lap Bands. (Toronto, Ontario)
