I also have UHC insurance, I didn't have to have that.
When I filled out the paper work at the docs office, I had to list diets I have tried at and failed the last 5 years and that is it.
I would suggest you call UHC yourself and ask if they cover Lap Band (the CPT code is 43770), also make sure your employer does not have an exclusion for WLS.
When I call UHC, they said the only thing that was requried was a BMI of 40. I was approved within 2 weeks.
