I would definitely get a copy of the specific criteria your insurance requires in order for the surgery to be medically necessary. And when you call the insurance company be sure to make a note of the date and who you talked to in case you have to fight them later. My insurance wanted a medically supervised diet for at least 6 months. I had been doing that loosely for over 2 years with my primary care doc. All I did was send in my records that suggested I eat low carb or that we talked about weight loss and prayed that they would accept it. Luckily, that was all that was needed as far as the diet. I have Aetna, but I'm sure that all insurances are different so make sure you get eveything (and then some) together before you submit it to your insurance. Good luck!
