Insurance appeal forms?

Hello, I have looked everywhere for an insurance appeal form. I was denied coverage for the LAP-BAND® and I am going to appeal it. I just don't know the right kind of page/form to send in with all of my records. I have tried obesityhelp.com and some other sites with no luck. If anyone can point me in the right direction I would appreciate it. Thank you.

Check out the inamed web site, I think there might be some info on there or call their customer service....Also, do a quick search on this web site for appeal letter examples. There are some good ones out there....

Kittylover, there isn't any one "form" to use for an appeal. If your carrier has specific procedures they want followed, you'll have to get that information from them. Generally an appeal is just a letter with any additional information that wasn't provided earlier, and it's VERY important that you know exactly why the initial request was denied. Your appeal should address the precise reasons that the carrier denied you in the first place.

340 highest / 210-ish lowest / 275-ish now / less
