My insurance wants a list of comorbidities. To my knowledge, I do not have diabetes or any of the other more obvious comorbidities. I do have plantar facitis (sp?), tendonitis in my knees, incontinence, lowered sex drive - that type of thing. Do these count? Do you have any examples of other comorbidities? Chronic Fatigue Sydrom? I really want to be thourogwhen filling out my medical history form.

Here is a link from from e-medicine.com HERE. In section 3 of 11.
I read their terms of use, so I didn't want to cut and paste the section - but there is lots of good information there.

GERD (gastroespohageal reflux disease), Urinary stress incontinence, hyperlipidemia/hypercholesterolemia, degenerative joint disease, low back pain, shortness of breath with exertion (exertional dyspnea), asthma, gallbladder disease, lower extremity edema (swelling), venous stasis (The blood pressure in the lower legs then increases, causing swelling, thickening of the skin, and sometimes ulceration of the skin)...talk to your PCP about all of these. Good luck and don't forget there are possible free appeals available to you if your LAP-BAND® is denied. You DO NOT have to jump through all the insurance company hoops before getting approved!

I have UHC Choice plus and we were approved within 30 days. I can forward you the letter that I had written for my pcp and my wifes OBGYn and they sent this to the LAP-BAND® doctor and he submitted and were approved. The biggest problem we had was they said they did not have my wife stuff. So we sent it again, and the third time it was faxed while the doctors office was on the phone and then she was approved within 20 minutes while they waited on the phone. She is having her done ong the 12 and I on the 19th of Sept. If you would like I can e-mail you the letter that I used. e-mail me at vcw61@excite.com or here.

:help:I have UHC choice plus and I am going for a consult 10-3-07 Wednesday. How long does approval take? I can go back with Dr. visits for 4 years, but have been over 35 BMI for at least 7 years. I cannot get my proof from weight watchers, which was about 6 years ago, because they do not keep records if you pay weekly they said. My co-morbidities that I have now are, chronic fatigue, lower back pain, Abdominal Fat, diagnosed Fatty Liver disease, allergies, Shortness of Breath on exertion, heart Arrythmia's
I am none smoker and non drinker, Ever! What are the chances that I will be approved, especially if some of these have only been the last year? Any help would be appreciated. |

Nancy,
I am in a very similar situation to yours. I can only go back 4 years with my medical records too... and have the same co-morbs as you do (expect the liver one)... and UHC still approved my surgery within 3 weeks.
They didn't even ask for any further records. |

I have the same insurance, but my husband's employer does not cover WLS. You are so lucky. |
