I was banded by Dr. Evanson 8/4/2009. I had my first fill 9/15/2009. Anybody else out there banded by Dr. Evanson? Or at St. Vincent's Carmel Hospital.

Surgery date set for August 4 - 7/06/09
Insurance approved surgery - 6/22/09
Initial consultation - 3/30/09;
5' 4.75" 237 # BMI 40
