Where can I find a list of BCBS HMO approved Surgery Facilities?

In my packet, it states they only cover part of surgery at a "Anthem Approved Bariatric Surgery Facility." I have spent HOURS on their site trying to find a list of these facilities...I can't!

I don't just want to call every single surgeon in the area, of course I will if I have to, but does anyone know where I can find a list of these places?

Your insurance company should know. I would start with them first. They typicall have a booklet or online assistance.
