My doc does pretty much the same thing as what Ariel said. I lay down, cross my arms over my chest and do a partial "crunch". He presses on my abdomen to find the port, numbs the skin and then does the fill. I don't think he pulls out all the fluid at every visit, he usually tells me when he does; I think it's like every other fill, but anyway, you sit up, drink some water to make sure it goes through with no problems and if everything is good, out the door you go. Fluids for 24-hours and then mushies for 24-hours, and then back to solids.
If they should overfill you, you'll know when you drink the water and they'll take some fluid out right then. I only had that once and it was instant releif when they unfilled me a bit. Still was numb and didn't feel a thing. :)
