band slipage ect

I found out on Friday that my band has slipped and they need to go in and fix it. Does anyone have experience on this? I had my band put in 15 months ago and have lost 115 pounds. What could cause this? I am having a problem getting Medicare to cover the new operation. They covered it before. Any comments? I am having this operation in Richmond Va. At St Mary's hospital by Richmond Sur. Thanks in advance for your input.
