I'm self-pay and I had to have the same tests/requirements as those trying to get insurance approval.

attend seminar
psych eval
nutritionist consult
EKG
Different bloodwork tests
Lung Xray
Upper GI
Stomach Ultrasound
Letter from family doc recommending surgery
