I was self pay also and didn't have any kind of program fee. My fee included surgery, fills for 3 months, unlimited nutritionist visits, monthly support group and after 3 months my fills are $15. I don't think I would have gone with a surgeon who charged a "program fee". Seems kinda shady to me! |
